To make your registration process quicker at your first session, click on the three icons below for each form. Print each one, complete each form in the comfort of your home, and bring it with you to your first appointment.

However, if you are not able to complete this at home, don't worry! Please let me know and we can arrange for you to come in 15 minutes earlier prior to your initial session to fill out the necessary documents.
